Prurigo actínico

Resumen

Descripto en la década del 50, el prurigo actínico (PA) es una fotodermatosis infrecuente que afecta en la mayoría de los casos a nativos y a mestizos de Sudamérica y Centroamérica, cuyo tratamiento representa un gran desafío. Múltiples factores epidemiológicos, geográficos, clínicos e inmunológicos están involucrados en su etiología. Afecta más a mujeres, suele iniciarse durante la primera década de vida, comprometiendo de manera simétrica la piel de áreas fotoexpuestas, así como también la semimucosa labial y la conjuntiva. Clínicamente se presenta con pápulas eritematosas, nódulos, liquenificación, híper o hipopigmentación post inflamatoria y cicatrices, acompañados por un prurito intenso que afecta profundamente la calidad de vida del paciente. El diagnóstico se realiza con la clínica y con la historia personal y familiar del afectado. El PA representa una respuesta anormal a la radiación ultravioleta A y B; se lo asocia con el complejo mayor de histocompatibilidad HLA-DRA (DRB1 0407 Y DRB1 0401).

Abstract:
Discovered in the 1950s, Actinic Prurigo (AP) is an uncommon photodermatosis affecting in the majority of the cases, natives and half-breeds of South and Central America. Its treatment represents a major challenge. Multiple epidemiological, geographical, clinical and
immunological factors are involved in the etiology of this entity. Most commonly found in women, it usually starts during the first decade of life, compromising symmetrically sun exposed skin areas, as well as the semimucosa of the lips and the conjunctiva. Clinically, it presents with
erythematous papules, nodules, lichenification, hyper or hypo post-inflammatory pigmentation and scarring, accompanied by an intense pruritus, deeply affecting the quality of life of the patient. The diagnosis is made primarily by the clinical aspect and both personal and family history of the affected patient. AP represents an abnormal response to ultraviolet A and B, and it is associated with the Human Leucocyte Antigen system HLA-DRA (DRB1 0407 Y DRB1 0401).
