Hidradenitis suppurativa is a chronic, recurrent inflammatory disease that compromises the skin with apocrine glands in armpits, inter and submammary, inguinal, genital and perineal areas. Overall prevalence has been estimated around 1% of the population. The disease has a significant negative impact on the quality of life of those affected compared to the general population. In advanced stages, in which medical therapy is insufficient, the dermatologist must determine what type of surgical approach to perform considering different alternatives, providing good functional and aesthetic results. We present the case of a patient with hidradenitis suppurativa stage III in who besides medical measures, extensive surgery and healing by second intention have been used obtaining an excellent curative, functional and an acceptable aesthetic result.
