Scalp paradoxical psoriasis during treatment with secukinumab

Paradoxical psoriasiform reactions include new cases or worsening of psoriasis, during treatment with biologic agents. Anti TNF-α drugs, including adalimumab, are the most common triggers for these reactions. The disruption of the balance between TNF-α and interferon-α would be key in its etiopathogenesis. However, involvement of the IL-23/IL-17 axis has also been observed, and other agents, such as secukinumab, have been implicated in the development of these reactions. We report the case of a patient with hidradenitis suppurativa receiving secukinumab who developed a paradoxical psoriasiform reaction on the scalp.
