Suppurative umbilical lesion in a previously healthy patient

An 86-year-old female patient, with a history of ischemic stroke in 2011 and senile dementia, presented to our service with an umbilical lesion of 8 weeks' duration. Physical examination revealed a 1.3 cm diameter, slightly painful, erythematous, infiltrated nodular lesion with serous drainage, located in the umbilical region (Photo 1). Dermatoscopy showed some milky red and whitish areas, punctate vessels, and serous drainage (Photo 2). Adjacent to this lesion was a second 0.5 cm diameter, infiltrated, erythematous nodular lesion with a smooth surface, present for one month, which had a shiny, non-secreting surface and was marked by telangiectasias (Photo 3). During targeted questioning, the family member present reported asthenia and weight loss associated with the lesions, of 2 months' duration.
