Mucosal melanoma (MM) is a rare and very aggressive variety of melanoma, representing less than 1% of these tumors. Due to its location in areas of difficult access, diagnosis and treatment are generally more complicated. We report a case of a 73-year-old-man with epistaxis, left nasal obstruction, proptosis, epiphora and ipsilateral diplopia for 2 months, with evidence of tumor lesion in the left nasal cavity compatible with MM. He started treatment with pembrolizumab, with a good response.
