Angiokeratomas are well-defined benign vascular lesions, consisting of superficial vascular dilation and hyperkeratosis. Five variants are recognized and with the exception of angiokeratoma circumscriptum (which represents a capillary-lymphatic or capillary malformation), they are a consequence of ectatic dilation of pre-existing vessels in the papillary dermis. Clinically they present as red to bluish-black warty hyperkeratotic papules. Diagnosis is based on clinical presentation, dermoscopy and histopathology. We report the case of a 25-year-old man with the presence of purplish-black hyperkeratotic papules on his right feet since birth.
